Transportation Services Acknowledgement and Release Form

* I fully understand that United Care Link Transportation is a private transportation service company and they have the right to collect payment for services prior to services being rendered. I also understand that, United Care Link Transportation has the right to refuse service, as those decisions are always subject to availability and confirmation by United Care Link Transportation.

* I agree to release from liability, hold harmless, indemnify and waive my right to sue United Care Link Transportation, for all claims or damages, I separately or collectively may have, for personal injury, bodily harm, death, injury to or loss of property, emotional injury or loss of consortium, that may occur in connection with, arising from or by reason of this transportation agreement, whether caused by negligence or otherwise. I understand that I am not releasing United Care Link Transportation from liability for claims or damages arising from a reckless or intentional act of United Care Link Transportation.

* I understand that camera may be used. Conversations and video footage inside and outside of the vehicle may be captured in event of an accident or similar occurrence.

* United Care Link Transportation will not be held responsible for late or missed appointments or all other similar situations, caused by traffic, weather, or unforeseen circumstances.

* I will not hold United Care Link Transportation financially responsible for any food/drinks that may fall/spill while inside the vehicle. I further understand that any liquids or foods that may burn my skin/soil my clothes or personal belongings due to speed bumps, new traffic patterns, road closures/detours, sudden stops, weather conditions and/or road hazards or similar thereof, is my responsibility and United Care Link Transportation will not be held liable or responsible. I further understand that a cleaning fee of $75 will be charged for any food, bodily waste or similar left behind in the vehicle (s).

* Animals and/or pets of any kind are not permitted inside, United Care Link Transportation, vehicle (s) for any reason. Services animals with proper documentation will be evaluated on a case by case basis and a final decision will be determined by United Care Link Transportation, prior to services being rendered.

* A refund of services will be promptly dispersed in the event United Care Link Transportation are unable to fulfill their service obligations, due to hazardous road/weather conditions, vehicle damage/maintenance or similar.

* United Care Link Transportation does not guarantee or provide another form of transportation in the event our vehicle (s) incurs inoperable damage from an accident, hazardous road conditions, or similar.

* United Care Link Transportation requires full payment to secure my appointment date/time. Only appointments canceled at least 24 hours before the confirmed appointment date/time will receive a full refund of the monies paid. Appointments canceled within a 24 hour window of the confirmed appointment date/time are not eligible for receiving a full refund. $50.00 will be charged as late cancellation fee.

We serve Private Individuals, Caregivers, Discharge Coordinators & Social Workers needing safer alternative to Cabs/Taxis/Uber & more affordable than ambulances.

Wheelchair & Stretcher transportation pricing is not one-size-fits-all; it depends on your trip details like distance, level of assistance and time of service, plus a base fee. Can I book same-day or immediate ride?

Unlike taxis/Cabs or rideshares, United Care Link Transportation is a dedicated Non-Emergency Medical Transportation (NEMT) service, not an on-demand car service. To ensure your ride is safe, properly equipped, and staffed by the right team for your specific needs, ALL rides require a minimum of 24 hours advance notice.

Scheduling ahead allows us to confirm your vehicle type, assign a trained driver, and guarantee an on-time arrival, because your care is too important to leave to chance. We recommend booking as early as possible, especially for recurring appointments like dialysis, chemotherapy, or facility transfers.

Yes, we provide Out-of-State long distance transportation services ensuring that our passengers are comfortable and safe throughout the journey. It comes with an option for our Buddy-Companion Service, where we provide an attendant (Certified Nursing Assistant CNA) to accompany the passenger when necessary.
